New Antenatal Referral Form

Mater Mothers’ Hospital Antenatal Clinic has developed a new standard referral form that will be launched Monday 19 October 2009. The form has been developed in consultation with the Divisions of General Practice to improve the services the hospital provides.

What does it mean for GPs?

  • Mater Mothers’ Hospital Antenatal Clinic will only accept referrals on this form.
  • If referrals are received on other referral forms they will be returned to the GP with the correct referral form attached, with a letter of explanation.
  • If referrals are incomplete, they will be returned for completion with a letter of explanation.

Why have a standard referral form?

Mater Health Service is moving towards a standard referral for all specialities within its public health service. This standard referral form will eliminate the need to for us to re-contact you for clarity around named referrals or clinical information. We anticipate this will dramatically improve the speed with which we are able to process referrals to our service.

What do I need to do?

There are numerous ways you can refer to the Mater Mothers’ Hospital Antenatal Clinic.

  1. Fax or mail the new standard referral form by:
  • printing out (as a Microsoft Word document or PDF) and complete by hand and then fax or mail
  • completing the form in Microsoft Word and then print out and faxed or mailed
  • completing the form using the interactive PDF document and then print out and fax or mail.
  1. For users of Medical Director or Genie you can download the referral templates and instructions from this site. Many of the required data fields on the referral form will be auto-completed from your management system.

You are encouraged to start using the new standard referral form now for all Mater Mothers’ Hospital Antenatal Clinic referrals. Remember that as of Monday 19 October 2009 only referrals using the standard referral form will be accepted.

This is a significant change to referral processes and we acknowledge that these changes will cause some inconvenience in the short-term. We appreciate your support in this initiative to improve the timeliness of our response to referrals.
