Mater Aged Care in an Emergency

Mater Aged Care in an Emergency (MACIAE) is a Mater service dedicated to supporting aged care facility(ACF) residents, their families, facility carers and general practitioners (GPs); with the goal of providing a seamless care transition between primary and tertiary sectors in order to ensure the highest and safest standards of care with the utmost compassion and dignity.

MACIAE is facilitated by a specialist aged care clinical nurse consultant who co-ordinates with all key partners involved in the care delivery to this unique population, to optimise their health outcomes and and communication pathways between stakeholders. MACIAE aims to support the delivery of:

"the right care, in the right place, at the right time"

 

MACIAE provides four key functions

Clinical liaison and support

Telehealth chronic disease management

  • clinical telephone access is available for ACF nurses and GPs, when deciding whether to send a patient to hospital
  • Monday to Friday care is provided by the MACIAE clinical nurse coordinator, 8 am to 5 pm
  • After hours support is provided by the Mater Hospital Brisbane (MHB) Emergency Department
  • Telephone triage is provided under the clinical governance of emergency specialists or physicians as required
  • Access to Mater outpatient specialist appointments via Telehealth to assist with better management of complex health conditions, medications and coordinated care planning.
  • All Telehealth appointments are coordinated and attended by a Mater physician.

Hospital complex care coordination

Educational support

  • Central point of coordinated care planning for residents appropriately admitted to Mater Hospitals to support the care continuum through to discharge.
  • Educational support for aged care facilities may be considered in areas of clinical suitability to increase the skill mix in areas of relevant to the patient.

 

Clinical telephone support is available 24 hours, seven days a week. Complex care coordination and Telehealth specialist appointments are available Monday-Friday, 8 am to 4 pm. To request education support, please contact MACIAE’s clinical coordinator.
