Young Adult Shared Care pilot program improving integrated care for young people

Mater Young Adult Health Centre Brisbane (MYAHCB) has partnered with Brisbane South Primary Health Network (BSPHN) to deliver a 12 month pilot program aimed at shared provision of care for young people aged 16 to 25 years in the hospital and primary care settings.

The program will involve existing MYAHCB patients participating in a number of young adult friendly programs, connecting them with a GP if they do not already have one.

Young people will be referred into the service by Mater medical or nursing specialists based on their needs.

Benefits of the program include:

  • Better connecting young adults to GPs where there is a need
  • Educating young people on the role of GPs in their ongoing healthcare
  • Assisting with access to a Medicare card and My Health Record
  • Improving communication between hospital staff and GPs
  • Provide a direct line of communication for GPs to contact the Shared Care Coordinator
    • The hotline enables GPs to access efficient and effective care plan information for young adult patients from the hospital
  • Assessing and supporting young adults to understand the role of primary care when  ‘transitioning’ from paediatric health care into adult care
  • A young adult specific health care skills education program is currently in development and once finalised will be made available to GPs and allied health clinicians.
