Mater Young Adult Health Centre Brisbane (MYAHCB) has partnered with Brisbane South Primary Health Network (BSPHN) to deliver a 12 month pilot program aimed at shared provision of care for young people aged 16 to 25 years in the hospital and primary care settings.
The program will involve existing MYAHCB patients participating in a number of young adult friendly programs, connecting them with a GP if they do not already have one.
Young people will be referred into the service by Mater medical or nursing specialists based on their needs.
