Transitioning from paediatric to young adult diabetes care

Moving from paediatric to young adult diabetes care is a major step and the thought of changing teams can be daunting for patients.

The Mater Young Adult Health Centre Brisbane's Diabetes Clinic team is one option for the next phase of diabetes care. This service provides exceptional specialised diabetes care to young people aged 16 to 25 years. A dedicated team of healthcare professionals aims to provide the highest standard of clinical care while supporting the emotional, social and educational needs of its patients.

The centre is hosting a Young Adult Diabetes Transition information evening where children and parents can meet the diabetes team of endocrinologists, diabetes nurse educators, psychologists, dietitians and mental health specialists. Other parents and children who have transitioned in to the service will also be there to answer questions and help patients choose the next diabetes healthcare team to support their diabetes management.
