Discharge Summaries

We're creating a new Discharge Summary for Mater

Mater is committed to providing our patients, GP partners and care providers with information required to safely transition from hospital care back into the community.

As part of this commitment, Mater is designing a new Discharge Summary which will allow us to align with the National Guidelines recently released by the Australian Commission on Safety and Quality in Health Care.

Mater is also committed to greater utilisation of secure electronic messaging to facilitate more reliable and timely transfer of information to GPs. 

Mater anticipates that the new Discharge Summary will be ready for implementation later this year
