Mater Aged Placement Service (MAPS)

Who we are

Do your patients need support transitioning into a residential aged care facility? Finding the right placement for them or a member of their family can be an overwhelming experience. 

Based in South East Queensland, Mater Aged Placement Service (MAPS) have expertise across the health and aged care systems and will work with families to find a suitable aged care home and support this transition. Our team is made up of experienced professionals who work within the Australian Association of Social Workers code of ethics, alongside caring support staff who will assist with more practical aspects of the process. We understand the admission process of aged care facilities, and we have extensive experience in liaising with older people and their families.

MAPS accept referrals from within our hospital network and the wider community.

What we do

  • an assessment of your patients' aged care requirements
  • identification of which aged care facilities may be suitable
  • liaison with and referral to Aged Care Assessment Teams (ACAT), who are responsible for assessing eligibility for aged care placements
  • assistance with the completion of paperwork, including application and asset forms
  • ongoing liaison until a suitable aged care placement is secured.

Fees

MAPS is a private offering and fees will apply for use of this service. As not all placements are the same, and patients with more complex needs may require additional services.
