Mater’s SmartForm referrals are set to cease. Start transitioning your referral processes now.

Important changes to Mater’s SmartForm referrals

What is changing?

In order to consolidate and simplify referral avenues for uninsured patients across the state, Mater will be ceasing the use of our SmartForms from Friday 12 May 2023.

This change will impact public adult specialist referrals and antenatal referrals sent via SmartForms.

Why is the change occuring?

Mater is dedicated to supporting GP experience, communications and improving the quality of referral information for our clinicians. After a recent review of referral methods, Mater has decided to simplify processes by reducing the number of electronic referral portals and no longer accept SmartForm referrals. This change will improve safety and quality of care for our patients and enable our GPs to use alternative referral methods to reduce referral rework.
