National Disability Insurance Scheme (NDIS) Access

From 1 July 2018 patients residing in Mater Health’s catchment area are able to contact the National Disability Insurance Scheme (NDIS) directly to discuss their disability and the impact it has on their lives.

The NDIS is a new national approach that changes the way people under the age of 65 years of age who have a permanent and significant disability access support and services. 

As an insurance scheme the NDIS takes a lifetime approach—investing in people with a disability early to improve their outcomes later in life. 

The NDIS supports people with a disability to build skills and capability so they can participate in the community and in employment.

To access the NDIS, a person must:

  • meet residency requirements
  • be aged under 65 at the time they apply to access the scheme; and
  • demonstrate they have a permanent disability that affects their everyday life

If your patient is already receiving disability support services from their state they will be contacted as the NDIS becomes available in their area.

If they are not currently receiving any disability support services but wish to join the NDIS they or their guardian (including their carer or nominee) will need to complete an Access Request Form as the initial step.

This form can be obtained by directly contacting National Disability Insurance Agency (NDIA) on 1800 800 110 and asking for an Access Request Form.

Copies of a patient’s existing diagnostic reports and/or letters can be requested from Mater Health and sent to the patient's GP.
